VAX-D is a partially self-directed treatment plan
assisted by your medical director. It
is vitally important that you comply with all treatment aspects.
Upholding this responsibility will allow you to maximize your benefit.
A missed appointment without 24 hour notification
beforehand will result in a $50.00 charge. Two
missed appointments will result in treatment suspension until your situation can
be clarified by your medical director. You
will not be able to schedule further treatment sessions without clearance by
your medical director. Missed
appointments not only hurt your chances for VAX-D to benefit you the most, but
will also limit someone else in getting better sooner.
